PRI (Patient Review Instrument) and screen is a professional assessment and screening of all skilled nursing facility applicants. It is used as a 'benchmark' or universal assessment to determine appropriate level of care. The assessment results in a 'score' that corresponds with various levels of care and assures appropriate services to meet the resident's needs. A PRI is required by the New York State Health Department before admission to a skilled nursing facility.

A Patient Review Instrument (PRI) is a medical evaluation tool that identifies whether or not an individual is eligible for skilled nursing care placement. A Patient Review Instrument (PRI) Screen determines the level of care and type of facility needed. It will also factor into the reimbursement the facility will receive for providing care. The eight-page PRI form is completed by certified trained evaluators, usually a registered nurse. If the patient is in a hospital the hospital staff will complete the PRI. A PRI is required by law and is valid for 30 days.
