  The Jewish Senior Life Physician House Calls Program offers patients: In-home primary care and assessments of medical problems In-home acute appointments for urgent, non-emergency care for patients of the practice Coordination of care with other home health care services One-on-one visits with the physician Review of medications and methods of taking them Assessments of abilities to perform daily activities, such as dressing, bathing, walking and eating Assistance in developing a plan of care in the event that the patient's health worsens.
