Program New Vision Services:

New Vision Services

  • Description:

    Provides one-on-one services which are individually designed support services essential for the waiver participant's health and welfare and to enable the participants to live as independently as possible. Services include: oversight, supervision and assistance with daily living skills such as bathing, dressing, meal preparation, household tasks, transportation, shopping, socialization, task completion, communication, interpersonal skills, mobility, problem solving, money management, etc. Must have Medicaid and Diagnosis of TBI (Traumatic Brain Injury) between the ages of 16-64 or another disability meeting criteria for nursing home level of care. Covered by Department of Health (DOH), TBI and NHTD (Nursing Home Transition and Diversion) Medicaid Waivers.

    Provides DOH TBI & NHTD Services as well as private pay in-home care to residents of Chautauqua, Cattaraugus and Erie Counties.
